In order to help us make your visit to our office as efficient as possible, we offer the option of downloading and printing our PATIENT FORMS for you to complete prior to your first appointment. If you prefer (or don’t have access to a printer) you may complete the forms in our office. We ask that you arrive 15 minutes prior to your scheduled appointment time to do so.
To allow us to better care for you, please bring the following with you to your appointment:

Your insurance cards
both Vision and Medical

A list of your medications
including dosages

Any glasses that you currently wear
including prescription sunglasses and OTC reading glasses

Any contact lenses that you currently wear
bring the boxes/ images of the boxes, or a copy of your prescription if available
